Summit Salon Academy 04166800聽received education stabilization funds under Section聽 18004(a)(1) of the Coronavirus Aid, Relief, and Economic Security Act (鈥淐ARES Act鈥�), Public聽 Law No: 116-136. This Fund Report applies to the student portion received under the Higher聽 Education Emergency Relief Fund that is designated exclusively for emergency financial aid聽 grants to students.

The institution appreciates that Congress and the President have made these critical funds聽 available for eligible students who have expenses related to the disruption of campus operations聽 due to the coronavirus pandemic. We take receipt of these federal funds seriously and are聽 distributing them in accordance with the CARES Act and implementing guidance.

The institution is making the below information available for transparency purposes and in聽 compliance with the U.S. Department of Education鈥檚 (鈥淒epartment鈥�) Electronic Announcement of May 6, 2020.1聽For questions or concerns regarding this Fund Report, please contact聽Dana,聽 Financial Aid Officer,聽补迟听dpurnia@summitsalonacademy.com.

1. The institution signed and returned to the Department the Certification and Agreement [for] Emergency Financial Aid Grants to Students. The institution has used, or intends to use, no less than 50 percent of the funds received under Section 18004(a)(1) of the CARES Act to provide emergency financial aid grants to students.
2. The total amount of funds that the institution will receive or has received from the聽Department pursuant to the institution鈥檚 Certification and Agreement [for] Emergency聽Financial Aid Grants to Students is聽$50,380.00.
3. The total amount of emergency financial aid grants distributed to students under Section聽18004(a)(1) of the CARES Act as of the date of this Fund Report is聽$50,380.00.
   
4. The estimated total number of students at the institution eligible to participate in聽programs under Section 484 in Title IV of the Higher Education Act of 1965, and thus聽聽eligible to receive emergency financial aid grants under Section 18004(a)(1) of the聽聽CARES Act, as of the date of this Fund Report is 47.
5. The total number of students who have received an emergency financial aid grant under聽Section 18004(a)(1) of the CARES Act as of the date of this Fund Report is聽47.
6. The methods used by the institution to determine which students receive emergency financial aid grants and how much they would receive under Section 18004(a)(1) of the CARES Act are鈥�
   • The Academy will prepare a roster of eligible actively enrolled students as of November 20,聽 2020. The total student grant award amount for each eligible student will be determined by聽 dividing the Grant Fund equally among the roster.聽
   • Students that have filed a Free Application for Federal Student Aid (FAFSA) and for which the聽 Academy has a current Institutional Student Information Record (ISIR) will be considered Title聽 IV eligible. Students that have not filed a FAFSA may still demonstrate Title IV eligibility 补苍诲听 thus eligibility for emergency federal student aid grants under this policy upon proof of:聽
     • US citizenship or that the student is an eligible noncitizen;聽
     • a valid Social Security number;聽
     • registration with Selective Service (if the student is male);聽
     • a high school diploma, GED, or completion of high school in an approved聽 homeschool setting; 补苍诲听that student is not delinquent with respect to any outstanding federal student loans.
7. 聽Any instructions, directions, or guidance provided by the institution to students concerning the emergency financial aid grants are provided 补迟听Attachments A聽补苍诲听B.

Please return this attestation statement to the Academy as soon as possible but no later than聽 December 7, 2020. Failure to return this attestation statement by this date may result in your聽 grant award being forfeited and those funds being used to make additional grants to other聽 students.聽

I accept the grant award provided to me from funding made available to the Summit Salon聽 Academy under the CARES Act.

• I am eligible for Title IV Federal Financial Aid.
• I understand that any money awarded from this grant is to be used for expenses聽 related to the disruption of campus operations due to coronavirus. These expenses聽 include eligible expenses under a student鈥檚 cost of attendance in the calculation of聽 Federal Financial Aid, such as food, housing, course materials, technology, health聽 care, and childcare. The money I am receiving is intended to cover my expenses聽 related to the disruption of campus operations.

Dear Student,

The Coronavirus Aid, Relief, and Economic Security (CARES) Act provides emergency聽 assistance for students who are in need of financial support for expenses related to the disruption聽 of campus operations due to the coronavirus pandemic. Under the CARES Act, the Summit Salon Academy has received grant funding from the U.S. Department of Education for the聽 purpose of making emergency financial aid grants to its students.

Enclosed please find an emergency financial aid grant check in the amount of聽$1071.92.听This is a聽 grant, which means that this amount聽does not need to be repaid. Your acceptance of these grant聽 funds is conditioned upon your agreement to use the funds for the purposes allowed under the聽 CARES Act and as agreed upon by you in the attestation statement that you previously provided聽 to the Academy. In particular, you understand that the money awarded from this grant is to be聽 used for expenses related to the disruption of campus operations due to coronavirus. These聽 expenses include eligible expenses under a student鈥檚 cost of attendance in the calculation of聽 Federal Financial Aid, such as food, housing, course materials, technology, health care, 补苍诲听 childcare. The money you are receiving is intended to cover your expenses related to the聽 disruption of campus operations.

Summit Salon Academy Student Emergency Financial Aid Grants Reporting聽

• The academy will distributed to qualified students all the funds, $50,380.00 allocated by the Emergency Financial Aid Grants under Section 18004(a)(1) of the聽 CARES Act.
• Of the 47 students active and considered to qualify for the grant, 47 qualify.
